An employee climbed onto a truck to raise its boom and secure it. He operated the lower controls of the bucket while crouching on the toolbox walkway. After raising the boom, he stood up, turned to take a step forward, and slipped. He fell off the side of the truck and the extended outrigger struck his right leg, breaking the tibia and fibula.

An employee was overseeing the operation of a rope being respooled onto the drum of a wire winding machine. He went to remove a piece of wooden reel from the rope. His glove got snagged and his right hand was pulled into the roller, pinching his fingers between the machine's roller and the rope. The middle and index fingers were crushed.

An employee was assisting a boom truck operator with hooking a lifting chain to a road plate. As the employee was attaching the chain from the road plate to the boom truck crane hook, the boom moved down and the employee s left little finger got caught in the chain. The employee suffered crush injuries and an amputation of the finger between the nail and the last knuckle.

An employee was working on a utility line installing new equipment. The employee was attempting to change the battery on an impact gun when the employee's left hand contacted the tail of the conductor, causing electrical burns to the hand. Gloves were not worn at the time of the incident.

An employee was stringing new rope from a drum onto a pulling machine. The rope started to veer off to one side; the employee was pushing the pulling block back into place with his right hand when his neoprene glove was pulled into the block. The skin on his right thumb tip was torn.

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.
