An employee was packing and stacking boxes onto a pallet. As the employee walked past the machine, her foot was caught on a guard, causing her to trip and fall to the floor. The employee was hospitalized with a bruised hip and leg, as well as a vertebral fracture.

An employee was performing maintenance on a machine when the machine cycled while his right hand was in it. The employee's hand was caught between the machine and the shear blade inside of it, resulting in a cut to the bone on his palm.

An employee was performing maintenance on a napkin folding machine that included a bandsaw blade. When the employee pulled open the upper band wheel guard using their right hand, the employee's gloved fingers contacted the stationary bandsaw blade. The employee suffered a right middle fingertip amputation and a laceration requiring stitches to the right ring finger. The machine was not locked out and tagged out at the time.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
