Federal OSHA recorded a severe workplace injury at South State, Inc., Rt 73 and Kresson Road, VOORHEES, NEW JERSEY 08043 on — Cerebral and other intracranial hemorrhages, affecting the brain.

An employee was paving a section of roadway when a passenger vehicle entered the work zone and struck the roller the employee was operating. The employee was ejected from the equipment and suffered a broken clavicle and a brain bleed.

An employee was setting a steel beam in a cofferdam when it dropped onto his leg, resulting in a fracture.

SOUTH STATE INC.

Three sections of concrete forms had previously been assembled and used to pour concrete footings. The forms were being moved to prepare for the next section of the footing pour. One of the end sections was removed and hoisted away. The support braces of the next form were removed before the rigging was placed on the form, resulting in the form falling over and striking an employee's leg just above the knee. The employee's femur was fractured.

An employee was installing pillars while on a ladder when they fell into a cofferdam below and had to swim to a ladder to exit the dam. The employee suffered a fractured leg.

An excavator bucket struck an employee in the head, causing the employee to fall off a jetty and land on a rock. The employee suffered a broken leg.

An employee was splicing piles when the upper pile, which was suspended by a crane, broke free, swung, and struck the employee in the back, resulting in broken vertebrae tips and requiring hospitalization.

An employee was sitting on the bed of the work truck while picking up safety cones in a work zone. An SUV drove into the work zone and struck the work truck. The SUV then hit the employee's legs, knocking her to the ground. The employee was hospitalized with a leg injury.

On July 3, 2023, an employee was putting ice pallets away with a forklift when another forklift collided with the employee's forklift and crushed their left thumb, resulting in an amputation.

An employee was operating a riding lawn mower inside of a construction zone on the highway before new asphalt was laid. A vehicle entered the closed lane, striking the employee. The employee sustained blunt force trauma to the body, abrasions to the arms and back, lacerations to the head, and an open leg fracture.

Two employees were setting up a traffic control zone while another stayed in an attenuator truck. A vehicle entered the zone and collided with the back of the truck, and the employee inside it suffered a back sprain.

An employee was operating a riding blower machine on a road median when they were struck by a vehicle and ejected from the machine, resulting in ankle and shoulder injuries.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee fell while stepping off the tailgate of a pickup truck. The employee landed on the ground and suffered a broken left fibula and tibia.

Circle H Contractors, LP

On October 7, 2025, employees were conducting trenching operations for the installation of a new sewer line system. The excavator's bucket was detached and put inside the trench as it was being exchanged for a different-sized bucket. An employee entered the trench with a shovel to move some dirt located at the front end of the pipe. The excavator bucket shifted and slid, striking the employee s right foot and trapping it against the ground. The employee sustained a fractured right tibia.

WPM-Southern, LLC

Employees were working to dislodge a large truck that was stuck in a rut using tow straps and a front-end loader with the bucket removed. An employee connected the straps to the loader and truck and another employee slowly engaged the loader to close the coupler. The injured employee's arm became caught in a pinch point. The employee was hospitalized with a fractured arm.

Garrison Enterprise Inc.

An employee was lifting a piece of 30-inch line stopping equipment using a jack screw. A lifting eye weld snapped and the load dropped. The end of the jack screw then landed on the employee's heel and crushed it. The employee was hospitalized.

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.
