An employee was cleaning and servicing a slip and drive machine. While he was checking whether it was working, its gears pulled in a rag and then his right index finger. The finger was amputated at the knuckle.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

Climate Pros, LLC

An employee was using a grinder to cut benches for a condenser change-out. The grinder kicked back and lacerated the employee's left hand above the knuckle, cutting tendons. The grinder's cutting wheel was unguarded at the time.

An employee was moving dies on a hydraulic pan brake closer together so he could bend pieces of sheet metal. The employee's left index, middle, and ring fingers were crushed in the machine, and all three fingers were amputated.

An employee was performing preventive maintenance on a dryer at a commercial laundry facility. The emergency stop was released, and the dryer broke his left humerus and lacerated his left forearm.

Steam and hot water discharged onto the employee's upper body. He was hospitalized with second-degree burns to his face, neck, chest, arms, and hands.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

NYU Langone Hospital-Brooklyn

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

VILLAGER CONSTRUCTION, INC.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
