An employee was cleaning the bacon department by spraying the floor and two running bacon conveyor belts with a hose and hot water. The employee placed her hand on the side of the conveyor to remove the slippery layer of bacon residue. Her left index and middle fingers contacted the conveyor belt next to a roller and was pulled into a structural part of the bacon press that had a metal edge. The employee sustained an amputation to her index and middle fingertips.

An employee was moving an incline conveyor in the sausage department when one of the wheels potentially hit a dip in the floor near a drain. The conveyor tipped over and landed on his left leg, fracturing the employee's femur.

The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

An employee was opening a filter housing in the potassium chloride production area and got sprayed with a hot solution of potassium chloride brine. The employee sustained thermal burns to their right hand and legs.
