An employee was working as a hammer forge helper in a crew of seven employees. As the hammer forge cycled, a piece of metal shot out from it and broke the employee's lower right leg.

An employee used the emergency stop to turn off the trim press. Hydraulic pressure released out of the cylinder, allowing the press to come down onto the employee's hand and amputate two fingers.

An employee was removing flash from a forged product using a long rod. The forge operator did not see the wand while it was within the press, cycled the hammer, and struck the wand. The wand then flew upwards, striking the employee in the right side of his face and resulting in a fractured jaw and cheek bone. The employee fell backwards and struck his head, resulting in a skull fracture.

An employee was doing a tool changeover on a table mill and changing the mandrel. His hands were on both wrenches with his right thumb was between the two wrenches as he worked to unlock the mandrel. The wrenches came together, crushing his thumb. The employee was hospitalized with a partial amputation of the thumb.

An employee was helping to remove tooling that was stuck in a forging press (a full-revolution mechanical power press). The press cycled, resulting in partial amputations to two fingers on his left hand.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.
