Final narrative
On May 13, 2022, an employee was engaged in a concrete pour for a building used for bulk salt storage. The employee set up scaffolding brackets and then placed a scaffolding plank onto the brackets. The employee then arranged for a concrete truck to dump concrete inside the forms that had already been installed. The employee stood on the scaffolding plank as the concrete was being poured in and used a shovel to pry on the rebar as it shifted when the concrete began flowing into the form. The shovel broke, the employee lost his balance, and fell to the ground. The employee was hospitalized for a broken right wrist and a shattered right ankle. Both injuries required surgery.
