An employee was replacing a large steel wheel on a machine using a bottle jack. While lifting the wheel, the jack came down onto the employee's hand, resulting in crush injuries. The employee's left middle finger was medically amputated.

A temporary employee was helping other employees separate bins that were stacked together. When a bin was separated from the stack and slid forward, the employee's right ring finger was pinched between the lip of the bin he was pulling out and the bottom edge of the bin that had already been moved from the stack. The fingertip was crushed and lacerated, and had to be medically amputated.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.
