An employee was using two rod wrenches to tighten a stand of rods as they were placed back down a hole. While wrenching the rods, the employee went to tighten the rod boxes. The employee lost grip on the wrenches and their left little finger was caught between the wrench handles resulting in amputation of the fingertip.
An employee was completing a well intervention. The system encountered a gas pocket while circulating the well. The gas pocket was pumped from the well bore to the open top tank that was being used to circulate the well. An ignition occurred, resulting in a flash fire that caused first-degree burns to the employee's arms.
