An employee was completing a well intervention. The system encountered a gas pocket while circulating the well. The gas pocket was pumped from the well bore to the open top tank that was being used to circulate the well. An ignition occurred, resulting in a flash fire that caused first-degree burns to the employee's arms.

An employee was working on the floor of a workover rig removing tubulars from the wellbore when they were struck by the power swivel, which pushed them into a handrail. The employee suffered a fractured collarbone.
