Final narrative
An employee was operating a debarker when a chain slipped into the roll cage. The employee attempted to kick the chain back onto the sprocket when his right foot got caught in the sprocket and chain, resulting in amputations of the tips of the big toe and index toe.
On February 24, 2016, at approximately 4:00 p.m., an employee placed a 64-inch post in the saw. The blade hit a knot in the post and pulled the employee's hand into the blade, amputating the left thumb tip.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
