An employee was aligning a brace on the landing gear of an airplane to bolt into place. The brace was moved and his right middle finger was caught in the bolt hole. The fingertip was medically amputated.

While walking toward a parking lot, an employee tripped on a forklift tine and fell to the ground, fracturing his hip.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee was installing a rail on a machine. The roller pack was locked up, causing the chuck to not move. While manually removing the chuck, the rails were damaged. As a result, both the roller pack and the rail needed to be replaced. A fixed magnet from above the rail pulled the rail up and rolled it, pinching the employee's thumb between the rail and the magnet when he tried to install the new rail back to the position. The right thumb tip was amputated.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
