At 4:03 p.m. on June 27, 2022, a forklift operator was loading a pallet of material onto a carrier truck when the truck pulled away from the dock. The employee fell off the dock, still in the forklift. He tried to jump out of the forklift and it struck him, breaking ribs. A second employee jumped off the dock to help him and suffered a knee injury.
