Final narrative
Two ERA Removal employees were working on a lift, removing iron brackets on the outside of a five-story building under a 3-foot-wide, cantilevered slab from the roof deck/area above the fifth floor. The block parapet for the roof was built on this slab. A third employee was working from a ladder on the fifth floor. The cantilevered slab from the roof with the block parapet sheared off the building and fell down three floors, damaging and collapsing the third- and fourth-floor cantilevered slabs. The two employees on the lift were hit with a large chunk of the parapet. The third employee jumped off the ladder onto the fifth floor. Two employees were hospitalized for contusions and lacerations to the head.
A mixer truck driver was delivering a load of concrete to a jobsite. The driver was operating the controls outside of his truck when a 6x10-foot fascia wall gave way. One of the concrete fascia panels struck the driver's right calf resulting in amputation of the leg below the knee.
An employee was receiving a 32-by-8 panel wall that weighed 3,500 pounds. As the crane was going up, the cable of one of the clanks became caught on the panel wall. The panel wall collapsed and struck the employee's right leg, resulting in right leg fractures and amputation below the knee.
An employee was moving conveyors in a warehouse to another location using rolling casters. The legs became detached from the bottom of the conveyor, and the employee's right index finger was caught in the conveyor pinch point, resulting in a fingertip amputation.
An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.
An employee was preparing to dump wood from a roll-off dumpster when a piece of timber fell from the dumpster and struck the employee in the head. The employee sustained a laceration to the scalp and three cracked vertebral discs.
