On May 15, 2020, an employee was carrying empty carboard boxes to place them in a metal crate used to transport the cardboard. After placing the cardboard on the metal crate, she turned to walk away and her pant leg became caught on a piece of metal wire sticking out about 1 inch from the crate's corner. She then fell on the floor and fractured her left hip, requiring hospitalization.
