Five workers of two employers (Wastewater Specialties, LLC and Westlake Chemical Lake Charles South) were hospitalized when a flash fire in a vessel occurred. Two employees were on an elevated platform welding a flange onto a 54-inch segment of process piping at the quench tower. Three other workers were engaged in a hydro blasting operation. Employee 1 suffered head trauma, lacerations, and burns. Employee 2 suffered a lower leg/ankle injury. The other workers sustained 2nd and 3rd degree burns to face and torso, and fractures to their arms and legs.

An employee was performing maintenance and repair work on a boat. When the boat was started up, it caught fire and exploded. The employee suffered second-degree burns to the face, hands, and legs.

While prepping a push-boat to move to another location, employees filled both motors with 2-stroke oil and put the cowlings back on the motors. An employee went to start the motors. The port motor started but the starboard motor hesitated. The employee tried again; the starboard motor started up but fire shot out and fuel that leaked caught fire. One employee was hit by the flames, and sustained burns on the neck, left hand, and both legs.

On June 22, 2020, at 10:50 AM, an employee was installing a fuel sender in a 31-foot boat's fuel tank. As he connected the fuel sender wires, a spark was created and ignited the fuel vapors. The vapors exploded and then caught on fire, burning the employee. He jumped down from the boat and other employees ran over to put out the fire. He was hospitalized with third degree burns on his hands and a concussion.

The injured employee was assisting another employee with fueling a push boat when fumes/vapors in the area were ignited by a spark from the battery post. The injured employee sustained flash burns.

A team of employees was moving boats from one slip in a marina to another using rope. An employee slipped while moving around in a boat and suffered a broken left femur.

An employee was at a guard shack checking in and directing guests. After motioning a camper forward, he turned to head back to the guard shack and the first rear tire of the camper struck his left foot and he fell to the ground. The second rear wheel went over his right leg, resulting in a fractured femur. The employee was hospitalized.

An employee helped to load garbage onto a boat. The boat bumped into a piling, causing the employee to fall in the water. The propeller of the boat lacerated his lower left leg from the knee to the foot.

An employee was washing a boat when they fell from the boat to the ground. The employee sustained a compression fracture to a vertebra.

An employee was using a sledgehammer to remove a large prop with a knocker that was attached to a boat when the sledgehammer broke and a metal chip flew into his eye. This resulted in a laceration to his eye that required surgery.

An employee was checking on a machine outside of the building and removed a cover to clean out a blocked area. His hand was caught by a rotary valve in a dust collection machine. The employee's fingertip was amputated.

An employee was working to clean a glue roller with a scraper tool. The tool and the employee's left thumb were pinched between two rollers, resulting in a partial amputation.

An employee was re-arranging boxes of pipe insulation on a pallet. One of the boxes fell from the pallet and knocked over an upright empty cylinder. The cylinder fell and crushed the tip of the employee's right toe. The employee's toe required surgical amputation.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.
