An employee was quality checking a cutting device on a paper machine. As he went to manually rotate the flat outer surface of the band motor, his right index finger contacted the cutting side of one of the blades, resulting in a fingertip amputation. The machine blades were guarded at the time.

WESTROCK CP, LLC

An employee was loading a 9,386-pound paper roll onto a peel station. The paper rolled off the station and the employee was caught between two rolls of paper, resulting in a pelvis fracture.

An employee was threading a paper winder. When the paper curled, the employee tried to straighten the paper and contacted the paper slitter, resulting in a partial amputation to their right index fingertip.

Employee 1 was replacing a bucket (480-volt breaker) while Employee 2 was observing from outside the arc flash boundary (36"). Employee 1 completed the work and was closing the door when an arc flash occurred. Employee 2 sustained first degree burns on the forehead and cheek and first and second degree burns to the hand. Employee 2 was hospitalized. Employee 1 was not injured.

An employee was loading a paper roll onto a machine using a pneumatic powered dolly. The employee's hand was caught between a stationary roll of paper on the floor and the roll of paper they were moving. The employee suffered an avulsion to the hand and was hospitalized.

On October 22, 2025, an employee was working to put a conveyor chain back on the track of a conveyor when their fingers were caught between the chain and the chain track, pulling their right hand into the sprocket. Four fingers were amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
