At 9:45 a.m. on August 15, 2022, an employee was performing final cleanup in a grain bin containing soybeans when a paddle conveyor caught his right leg. He suffered an amputation to the leg below the knee.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was helping clean the bag house. He checked under the auger in the dust collection system to feel for airflow. His right hand was sucked up into the auger blade, resulting in amputation of the middle finger to the distal joint. Surgery was required to amputate to the finger to the second knuckle.

On August 22, 2025, an employee was working to remove a blockage of grain from a spout in the basement. The gate was manually closed from the floor above to stop the flow of grain to the leg. The gate closed down on the employee, causing the amputation of their left middle fingertip.

An employee was walking with a pole hand tool when he slipped on fertilizer product that was on the floor and fell backward. The employee sustained fractured ribs and a collapsed right lung.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
