An employee was loading material into a shear cutting machine when they sustained an amputation to their right thumb.
At about 2:00 p.m. on March 14, 2025, an employee was setting up a threader machine. When the machine was restarted, a 15-inch Allen wrench (which had been used to tighten the die and was still attached) whipped around and struck the employee's left forearm. His arm was broken and he was hospitalized, requiring surgery.
An employee was setting up a mechanical punch press when the pedal was activated. The machine rotated and contacted the tip of his left thumb, resulting in a fingertip amputation.
