Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.

An employee was doing a daily inspection of the sifter screen on a mixer when the mixer line activated. The armature for the screen struck the employee's right wrist. Their wrist was cut and their hand was fractured.

An employee was being trained on a machine when a roller came down on their hand, resulting in a fracture and laceration.

An employee was doing a dry run during the setup of an ice cream machine. The machine indexed and the filler head amputated the employee's left middle finger above the last knuckle.

An employee was operating a forklift to return a scrap hopper after dumping material. After the employee placed the hopper outside the aircraft hangar door, the employee noticed the hopper rolling toward the doors. The employee exited the forklift to stop the hopper and grabbed the top of the hopper. During this time, the forklift started rolling forward and struck the hopper, pinning the employee s right hand between the forklift mast and the hopper and resulting in hand fractures.

An employee left a vehicle and walked to the front of it, between it and another vehicle. He tripped and fell, landing on his left hip and breaking it.

An employee was leaving a meeting at a customer site. As they were descending an interior staircase, the employee lost their footing on the last step and fell forward, landing on both knees on the concrete floor. The employee sustained a bilateral patellar tendon tear.

On November 9, 2023, an employee was picking up cones and equipment from the roadway in a work zone when they were struck by a motor vehicle. The employee sustained a fractured right leg.

On September 26, 2023, an employee was performing maintenance on an X-ray detection machine at an airport. While the employee was descending from a conveyance system to a three-step ladder, they fell to the ground, resulting in a compound fracture and a dislocated left ankle.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
