An employee finished her afternoon shift and was walking to her car on the gravel/dirt/grass walking surface. As she approached the exit gate, she tripped/stumbled and fell onto a nearby fence and the ground. The employee sustained fractures to collarbones or rotator cuffs on each side of her shoulder(s).

At 2:30 p.m. on April 15, 2022, an employee was working under a school bus when a piece of metal fell into his eye and lacerated it.

An employee was walking from the parking lot to the facility when she slipped on icy pavement and fell, hitting the left side of her head and resulting in a skull fracture and facial contusions.

Employee was stowing a wheelchair in the bus when she got her finger caught in a pinch point amputating the tip of the right index finger.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

On 6/23/2025, an employee was walking when her shoe caught on the flat tile floor and she struck the office door next to her desk. The employee sustained fractured shoulders and required surgery.

An employee arrived at a client's home to pick them up. They were walking to the the front door when they slipped on ice and fell on the ground. The employee sustained a fracture to their right leg above the knee.

An employee had been using a forklift to offload steel crates from a truck. The employee was closing the panel door of a crate that was unloaded and had been set aside as another forklift was pushing the crates when the employee's right index finger became caught between the panel doors of the crates. The finger was crushed and lacerated, requiring surgery.

An employee was walking in the yard when a yard truck struck them. It was raining/snowing at the time. They sustained a fractured leg and hip, as well as bleeding on the brain and collapsed lungs.

An employee was walking to the dock when their feet got entangled in some cables that were attached to the building and they fell to the cement floor sustaining a head injury.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

An employee was lowering the bucket of a track loader that contained a grade stake and the grade stake punctured his foot, resulting in hospitalization and surgery.

An employee was using a pike to move a pallet of sugar that was stuck in the powder racking. The employee fell backward, landing on his right leg. The employee's leg was fractured and he was hospitalized.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
