Final narrative
On September 30, 2022, at approximately 12:15 a.m., an employee was finishing the task of scrubbing the inside of a cabinet in the harvest floor basement. As the employee was closing the lid, the Clean-in-Place (CIP) system started. The employee let go of the lid and his left ring finger was crushed between the corner of the lid and the lip of the cabinet. The employee sustained a partial amputation of the distal phalanx on the left ring finger.
An employee used a hook to pry up a cast iron manhole cover to check if water was flowing through the drain system. The cover slipped from the hook and fell on his right middle fingertip, causing an amputation that included bone.
An employee was trying to grab a box of product to turn it around on a conveyor so the label on the box was facing the correct direction. Her gloved left hand slipped off of the box and was caught in the in-going nip point on the chain and sprocket, causing amputations to her left ring and pinky fingers.
