An employee was in the nose of a nearly-empty container moving boxed merchandise to the tail. Some merchandise was stacked above his head. While moving the multiple stacked boxes of furniture with the hand truck, the box on top that contained a chairside table (18x18x28, weighing 33 pounds) fell and struck the employee in the head. He fell to the floor, resulting in injuries to his head, neck, right shoulder, and both ankles. The employee was hospitalized.
An employee was placing wheel chocks on a trailer. The trailer moved when another employee entered it, causing the wheels to crush the first employee's fingers. The employee suffered a partial amputation to a thumb.
