An employee was rolling two 1,00-pound iron pipes by hand when one of the pipes rolled back toward him, causing his right arm to become caught between the two pipes. He sustained a right radius fracture and was hospitalized for surgery.

A maintenance employee was taking a master link out of a chain when his hand got caught between the chain and sprocket resulting in fractures to the left hand.

An employee was helping to clean up an iron spill. A 3-foot skimming rod was laid on top of the machine ladle of a casting machine. The rod had been previously dipped in the blacking liquid. The wet rod fell into the ladle, causing a reaction that ejected molten iron from the ladle. The molten iron contacted and burned the employee's left foot.

An employee was taking a trash cart to a dumpster when one of their fingers was pinched between the trash cart and the dumpster, resulting in a partial amputation.

ISS Facility Services Inc.

An employee was pushing flat shopping carts back into the cart tunnel. When unjamming the carts, part of the cart struck the employee's left shin, resulting in a laceration and bleeding. The employee was hospitalized.

An employee was pulling a cart backward in a storage area when his right little finger was pinched between the cart frame and an electrical box mounted to the side of a support column. The employee's right little finger was partially amputated.

An employee was helping to push a welding trailer out of the path of a truck. The trailer then rolled over a half-inch elevation change that rocked the trailer toward an electrical box. The employee's finger was pinched between the welding trailer and electrical box, resulting in a finger amputation approximately 1/2-inch from the end.

An employee was unloading packages from a trailer onto a cart. While moving the cart, it struck the employee's heel causing a tear to their Achilles muscle. The employee required surgery.

An employee was troubleshooting a shell core machine that had failed to raise the sand hopper into place for core making. The employee went to manipulate a switch and the machine cycled. The employee s left middle fingertip was caught between the roller-ball switch and the roller-ball switch target, resulting in a laceration that required surgical amputation to part of the distal phalanx.

On August 18, 2025, at approximately 7:40 AM, an employee utilized a pocket knife to cut banding on a new bundle of rod material. The knife blade bent during use. The employee was working to straighten the blade on a hard surface when the knife slipped, causing a lacerating their right wrist/forearm. The employee was hospitalized.

An employee was manually pulling drive belts to rotate a machine that needed repair. The employee's right index finger was pinched between a rubber drive belt and a pulley, causing an amputation to the fingertip.

At about 9:15 a.m. on May 14, 2025, an employee was using a bending press to bend metal into a U shape. The press caused an amputation to the employee's left middle finger.

Victaulic Company of America

An employee was running a test pipe for 12 feet and went to pull the pipe out of a roll grooving tool. As he was doing this, the tool's control pedal actuated, causing the tool to rotate. The employee's glove became caught and was pulled into the rollset, amputating about half an inch of his index finger.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
