An employee was opening cases near a forklift that was loading 700-pound totes of frozen meat into a tote dump. A tote slipped off the forks and fell onto the employee, causing them to sustain fractures to the tibia, fibula, nasal bone, axis, and lower/middle back.

At about 11:00 p.m. on February 26, 2022, an employee was on an A-frame ladder, trying to unjam cardboard from a depalletizer machine, when the cardboard came loose and she fell. She landed on her feet and suffered a broken right knee. She was hospitalized.

An employee slipped on spilled spaghetti and fell to the concrete floor, suffering a broken left hip. The employee was hospitalized.

An employee was performing a sanitation inspection on a machine and amputated their middle fingertip.

An employee's right thumb was caught between a turnaround conveyor and a product guiderail. Half an inch of the thumb was amputated.

An employee was inspecting an apartment to ensure it was ready for a new resident to move in. The carpet in the apartment had been recently cleaned and was still wet. As the employee went from the carpet to the tiled floor of the bathroom, they slipped and fell, resulting in a torn left hamstring.

An employee was delivering a letter along a rural carrier route when she stepped on an ice-covered snow drift, slipped, and fell to the ground. The employee sustained a right hip fracture that required surgery.

An employee was removing dough from the discharge at the bottom of a mixer when the scraper contacted his left index finger and amputated it.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
