An employee was working in a mill control room on a stationary mounted ladder. As he was ascending the ladder, he slipped and fell to the ground, resulting in fractures of the left arm and three left ribs.

An employee was performing a necropsy on an animal to determine the cause of death when the animal shifted, and the employee's knife lacerated their right forearm.
