On November 3, 2022, an employee was working in the receiving/shipping area. The employee was adjusting the tension of a conveyor belt while standing near a stairwell. The belt broke, causing the employee to fall approximately five feet onto the second step of the stairwell. He was hospitalized for rib fractures and a hand laceration.

An employee was walking to the lunchroom when they fell on the tiled floor and sustained a fractured right knee.

An employee was conducting periodic maintenance on equipment. His right ring finger was caught between the belt and a pulley on a high-pressure pump resulting in amputation down to the middle of the nailbed. The machine was locked out at the time.

On January 5, 2023, an employee was operating a ketchup bottle labeling machine that used a roller system to pump glue onto the labels. Their left hand was crushed in the labeler machine, resulting in a ring finger amputation.

On August 31, 2019, an employee was guiding a roll of film into a pizza sauce machine that was being lowered with an overhead hoist when the employee's hand was caught between the film roll shaft and the cradle of the pizza sauce machine. The employee pulled her hand back and received an amputation of the right index fingertip. The unbalanced film roll struck her on the right side of her face between the nose and cheek, causing blunt trauma to the face. She was hospitalized.

Electrical Contractors Inc.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

On June 20, 2025, an employee was grabbing boxes from a conveyor and staging them on a pallet. While they were picking up a box, the employee's right hand was caught between the roller and the belt of the conveyor line, resulting in a right ring fingertip amputation and a laceration to their right middle finger.

An employee was using a forklift to drop a bulk bag of coffee scraps into a dumpster. The forklift overturned and the employee was pinned under the forklift. The employee was hospitalized with fractures to their pelvis and femur.

On October 27, 2023, an employee was getting onto a forklift to diagnose a computer problem when a forklift at the exit side of flow racking pushed a pallet into the rack. This caused a pallet of corrugated boxes at the other end to fall approximately 15 feet and strike the employee. The employee was hospitalized with fractures to the ankle, rib, and potentially his back.

An employee was changing a roll of film in a fill and seal machine. As she reached over the top of the belt, her left index finger was caught, amputating the fingertip.

The injured employee was observing a temporary employee for hands-on certification of a standup forklift. The temporary employee was backing down the aisle when the forklift pinned the injured employee against the upright of a warehouse rack. The injured employee sustained a laceration to an eye, and hematomas to their chest and foot.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
