A road maintenance employee was helping to pick up lane closure equipment while in the basket of a truck. As the basket on the back was raised from the lowered position, the employee's right foot became stuck between the frame of the box truck and the frame of the motorized personnel basket. They sustained four fractures to the right foot.

An employee was connecting a towable arrow-board to a truck hitch. A car entered the construction zone and struck the arrow-board, knocking it into the employee. The employee suffered leg and facial injuries.

An employee was collecting garbage with an automated garbage truck. A hydraulic line ruptured and sprayed into the exhaust, igniting a fire. The employee sustained severe burns on her right arm, back, and face.

An employee was driving a rear-property machine across a bridge over a river when the bridge collapsed and the machine and the employee fell 16 feet into the river below. The employee suffered a broken ankle and broken ribs.

An employee was standing while providing care to a patient in the back of a moving ambulance. The ambulance made a turn and the employee fell onto wooden cabinets mounted on the wall of the ambulance. The employee suffered back trauma that resulted in hemothorax, that required medical treatment.

An employee working as a bus monitor stood up to aid a student. When the bus hit a bump in the road, the employee fell and hit their head and the left side of their body. The employee was hospitalized for a stroke and brain bleeding.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

Anderson Columbia Co., Inc.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Warner Brothers, LLC

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

Don Martin Corporation

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

JERSEY CITY MEDICAL CENTER

An employee tripped over a stool and fell on the floor, resulting in a left displaced mid-cervical femoral neck fracture.

APM TERMINALS PORT ELIZABETH

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.

Allied Waste Systems Inc

An employee assisted in cleaning material from a conveyor pit. After the pit was cleaned, the employee proceeded to replace metal safety plates to ensure other employees did not fall into the pit. While replacing one of the last plates, the employee mis-stepped and fell approximately 5 feet into the pit. The employee was hospitalized with back/side bruising, elbow bruising, bone bruises, and/or fractured ribs.

An employee was inspecting a breaker box and turning on/off a circuit breaker when he was electrocuted.

An employee was walking on the sidewalk. When they stepped off the curb, they fell to the ground, resulting in fractures to their tibia, fibula, and a metatarsal.
