An employee was on an extension ladder, changing the lightbulb of an exit sign in a school gymnasium. The employee finished and started to go down the ladder when his feet slipped causing the ladder to slide inward. The employee fell to the floor with the ladder. The employee sustained fractures to the right heel, left foot, and one or more vertebrae.
