A temporary employee was cleaning up her work station at the end of her shift. She manually rotated the turntable platform of the spring-actuated pallet carousel skid positioner to bring a pair of pliers within reach. Her left middle fingertip became pinched between the tubular steel frame and the turntable platform resulting in amputation of the fingertip without bone loss.

An employee was on top of a heat furnace line, tightening bolts on a motor. The employee's left ring and little fingers were amputated.

An employee was changing mud cups in the hopper cylinders of a concrete pump truck. The cylinders moved and three of the employees fingers were amputated.

Residual gas and built-up oxygen in the purge chamber caused an explosion, which blew the front of the furnace's door off its track and the hood/exhaust system off its rivets.
