Final narrative
An employee was working with makeup tongs (manual pipe tongs) to make them clasp. The top drive torque spun counter clockwise, causing the tongs to clasp and the snub line to pull tight. The entire pipe stand flexed and the pin end lost contact with the floor, skidding backward toward the injured employee. The tongs and stand of drill pipe struck the employee on their left side resulting in fractures to the left tibia, fibula, and ulna.

An employee was working to move drilling equipment onto a truck for demobilization. The equipment began to tip and the employee jumped from the cab of the excavator, resulting in a tibia fracture.
A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel.
