An employee slipped on ice in a driveway and fell, resulting in a right tibia and fibula fracture.
An employee was on an 8-foot scaffold conducting a repair activity on a vessel when a hot substance shot from an opening in the vessel, striking the employee's face and causing second degree burns. The employee also fell off the scaffold and suffered a swollen right wrist.
