Final narrative
An employee was guiding film into a pouch-filling machine while the machine was jogging and the employee's right index finger contacted the cutting blade, resulting in a fingertip amputation.
Two employees were building pallets using reworked product. They collided as one of them stepped back and the other turned around. The injured employee fell on his right side, sustaining injuries to the right knee and hip. He was hospitalized and underwent surgery.
An employee was unjamming a machine when the machine blades amputated her right middle and ring fingers at the first knuckle. The machine was guarded at the time.
On July 24, 2021, at 4:15 p.m., an employee and a co-worker were verifying prior work done on a machine. The employee had his hand in the freezer bar sealer area when the co-worker activated the machine by opening the air valve that powered the sealer. The sealer then closed on the employee's right hand, resulting in an open fracture and an amputation of the middle fingertip.
