Two employees were doing bridge work. While removing rigging from a trestle cap, the trestle cap fell over and struck both employees. Employee 1 sustained fractured ribs and lacerations; employee 2 sustained a laceration.

An employee was assisting with replacing a cable on a boom hoist. He was working near a bridle, to assist the cable through the horizontal sheaves, when his gloved left little finger was caught between the cable and a vertical sheave, resulting in amputation.
Three employees were working on a bridge while in a mobile working platform. The platform fell, and the employees fell with it to the ground about 16 feet below. One of them suffered a broken leg and was hospitalized, requiring surgery.
An employee was working with a crane when a bolt broke and the cheek plate fell off and struck his left leg. His leg was fractured in two places and required surgery and hospitalization.
An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.
