An employee was removing product from the fondant wheel pump when their right middle finger was amputated.

On July 30, 2021, at 6:00 p.m., an employee was removing a hose and the stainless steel hardline for a positive displacement pump when their left middle finger caught on a blade in the pump, resulting in amputation of the fingertip.

Two employees were operating forklifts in a freezer. One forklift stopped to let the other go by. One of the forklifts slid on the icy floor and its forks struck the left calf of the other forklift's operator, fracturing their shin.

At about 4:30 a.m. on February 17, 2021, an employee was cleaning underneath a running conveyor. The employee's right glove was pulled into the conveyor as the employee was cleaning flour from the surface of the conveyor, causing the right hand and forearm to become caught between the conveyor and the surface underneath. The employee suffered a broken wrist/forearm.

An employee had been unloading cheese from a blender. The employee was holding two buttons used to control the cheese blender paddles/manually jog the equipment. The employee let go of the buttons and went to grab a piece of cheese that was stuck in the blender. There was a 4-second delay after letting go of the buttons before the paddles completely stopped moving. The employee's hand contacted the still moving paddles. The employee sustained an amputation to the corner of the left pinky fingertip and a laceration on the pad of the left pinky finger.

An employee was changing a saw blade when the saw machine activated. The employee's left ring and little fingers were amputated.

An employee was clearing a jam from the label cutting machine when their left middle finger was partially amputated.

Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.

An employee was cleaning the conveyor on a piece of equipment when they slipped and their right hand was pulled into the chain sprocket. The employee's fifth fingertip was amputated.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was attempting to straighten a wooden pallet with a bar. The pallet was positioned on top of a frame conveyor system 1 foot off of the ground. While maneuvering the bar, the employee lost balance and fell backward onto the concrete floor, extending their right hand to catch themselves as they fell. The employee suffered two fractured bones in their wrist and one fractured bone in their right hand.

An employee was working near a conveyor when his left middle finger was caught in the conveyor. The employee required surgery and their finger was partially amputated.

An employee was filling a hopper that deposited blueberries in muffin trays. Two plastic plates inside the equipment closed, pinching and amputating the employee's left ring fingertip. The equipment was running and unguarded at the time.

An employee was returning a catch pan to a machine after dumping fruit out of it. The employee's hand was caught in a roller in the conveyor area of the fruit inspection line, and the employee's right arm was broken.

An employee had been using soap and a towel to clean a slicer. They were finishing the cleaning procedure when the power switch was activated. The slicer turned on while the employee's right ring finger was in the blade gap and the blade amputated their fingertip without bone loss.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
