The employee let go of the buttons and went to grab a piece of cheese that was stuck in the blender. There was a 4-second delay after letting go of the buttons before the paddles completely stopped moving. The employee's hand contacted the still moving paddles. The employee sustained an amputation to the corner of the left pinky fingertip and a laceration on the pad of the left pinky finger.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.
