Federal OSHA recorded a severe workplace injury at Lexicon, Inc., 8900 Fourche Dam Pike, LITTLE ROCK, ARKANSAS 72206 on — Concussions, affecting the brain.

Employees were erecting structural steel. The injured employee was sitting on a 12" wide horizontal member and lining up an I-beam brace as it was suspended by an overhead crane. He leaned left to push the end of the brace. The end of the suspended brace swung away from him, causing him to lose balance and fall 8 feet. His personal fall arrest system arrested his fall, but he swung into a piece of duct work during the fall that resulted in a concussion.

An employee was using a sleever bar to move a checker plate into place and secure it to the floor of a structure in the melt shop. The sleever bar became stuck and the employee used his left hand to lift the checker plate up enough to free the sleever bar. Once the bar was freed, the checker plate came down and pinched his left index finger between two pieces of plate. The employee suffered an amputation of soft tissue at the fingertip, which required hospitalization.

On May 16, 2025, an employee crossed over a stand to support a wide flange beam they were fabricating. The beam rolled over onto their lower right leg and foot, resulting in fractures.

An employee was placing a steel I-beam when it became stuck and would not settle onto the flanges. The employee was moving the beam and when it became unstuck and crushed the fingers on his left hand. The employee was hospitalized and required surgery.

LEXICON, INC

The injured employee was assisting a crew to install a crane rail on an overhead crane girder. He was on the 40' x 10' grated surface between the crane rails positioning himself to receive the end of the crane rail. The crane rail was being pushed on rollers toward the employee's location and as he tried to pry the end of the crane rail up to get it on top of the next roller, he lost his balance and fell 36 feet to the ground below. The employee sustained fractures to their right ankle, pelvis, and left arm. The employee was wearing a personal fall arrest system, but was not tied off at the time.

An employee was installing drive shafts on an overhead crane. The drive shaft was rigged about 17 feet up with a wire rope choker. The drive shaft began slipping through the rigging, when it slid down and struck the injured employee's hard hat. He was knocked to his knees and landed on his right knee. The employee sustained an amputation to the right leg below the knee.

Peabody General Contractors

An employee was walking along a trench inspecting the trench box in the excavation when he tripped and fell toward the trench. As the employee was falling head-first into the trench, he attempted to get a hold of the fall protection cable. The cable then looped around his left hand and the employee fell approximately 8 feet into the trench before the fall protection engaged. The employee suffered amputations of his left index, middle, ring, and little fingers.

National Park Service

A ranger was performing back-country patrol duties while hiking. As they were climbing a technical rock patch, a rock handhold broke, causing the employee to fall approximately 15 feet before their belay engaged. The employee contacted a rock, resulting in a fracture to the right ilium as well as a subcutaneous hematoma to the right gluteal region.

On September 12, 2022, an employee was on a metal roof, installing a standing seam and insulation. He lost his balance and fell toward the third floor slab, which was 10 feet from the roof. His fall protection caught him and left him suspended about 2 to 4 feet from the slab. He suffered a brain bleed and a spinal fracture in his lower back. He was hospitalized.

West Tree Service, Inc.

An employee was working from a bucket lift, removing a tree in sections. He lost his balance while pushing on a section and fell. His fall protection caught him and he swung into a tree, suffering an injury that caused head, neck, and leg pain, as well as loss of consciousness while he was at the hospital.

Griffith Steel Erection, Inc

An employee fell from a steel beam. His fall protection caught him, but the momentum swung him into a concrete block wall. His head struck the wall and he was knocked unconscious, suffering a skull fracture and a brain bleed.

Burke Builders Inc

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

Hobby Lobby Stores, Inc.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

Cross Management Corp.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

STEEL STRUCTURES AMERICA INC

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

Trejo Concrete & Steel, LLC

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

NATIONAL MILL MAINTENANCE, LLC

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

The Goodyear Tire & Rubber Company

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

Big River Steel, LLC

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

American Tile & Marble Co., Inc

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

Johnson Industrial Coatings, LLC.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
