An employee slipped and fell while climbing onto a trailer. He hit his left lower leg on the guardrail to a scale, breaking it.

An employee was attaching an inspection onto a conveyor screw when a cooker gate opened and spilled hot oil that caused second degree burns to his neck and back.

An employee was checking equipment and adjusting lines. As the employee was unplugging a plugged line that uses steam, a handle was bumped releasing steam condensate (300 degrees) that struck the employee's chest causing a second degree burn.

An employee was working on a machine with a pry bar when the bar slipped, causing him to lose his balance and step backward into a sub-pump hole filled with hot water. The employee sustained 2nd degree burns to his left leg and was hospitalized on March 18, 2016.

On 11/24/15, at 1900 hours, an employee was operating a rough terrain forklift on a steep grade. The forklift toppled over and pinned his left arm underneath. He was hospitalized with a broken arm and had surgery.

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.

An employee was exiting a trailer for it to be loaded when their right ankle was pinned between a piece of angle iron that was anchored to the floor and a long pallet being pushed by a powered industrial truck (PIT). The employee sustained an ankle fracture and was hospitalized.

A tow truck operator was operating his bed controls on the drivers side when another vehicle crashed into him. The employee sustained fractures to the right femur, hip, and left foot, as well as abrasions to the right knee and a laceration to the right wrist.

A temporary employee was working to unclog butter from a butter melter pump. His right index finger was caught in the machine, and the fingertip was amputated. The machine was energized at the time.

An employee working at the reject conveyor was placing a product guide bar on the conveyor. This metal guide bar is placed on the side of the conveyor belt to funnel the product, preventing it from falling off the sides. As he was getting the attachment positioned, his left ring finger was caught between the conveyor belt and the conveyor belt frame. The employee's finger was fractured and partially amputated.

An employee was using a pneumatic impact wrench to tighten bolts on a brake press anvil. The impact wrench hooked up on something, causing the tool to rotate. This spun his left hand with it. Their index finger was pinched between the side handle of the tool and an anvil bolt on the press, resulting in an open fracture. The employee required a medical amputation of the fingertip.

An employee was cleaning the area under a mobile elevated work platform. The platform lowered and struck the employee, resulting in a fractured right femur. The employee was hospitalized.

As an employee was opening an infeed valve. Hot tallow oil in the line above fell back down the pipe and came out of an open pressure relief valve, striking the employee on the thighs. The employee was hospitalized with second-degree burns to his thigh. Another employee sustained burns to their back but they were not hospitalized.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was cleaning out corn from a grain bin. The sweep auger in the bin was activated and caught his trouser leg. His leg was pulled into the auger, which injured his calf and caused leg fractures and lacerations.
