An employee was loading a mill machine onto a flatbed trailer. The machine went into crab-walk mode and walked sideways off the trailer. The employee jumped from the operator's platform of the machine to the deck of the trailer. He suffered a broken right arm, five broken ribs, a bruised lung, and lacerations to the kidney and spleen.

An employee was flagging traffic in a construction zone when he was struck by a pickup truck. The employee sustained two broken teeth and lacerations to his face and right leg. He had surgery on his right leg.

At 4:19 p.m. on July 1, 2022, an employee became overheated while raking asphalt to patch holes on a highway. The employee was hospitalized, suffering from heat exhaustion.

An employee was on the catwalk of a paver, trying to dismount on the right side, when he was struck by an oncoming truck. He suffered burns (from hot asphalt) to both palms, the right elbow, the upper right temple, and the right ear, as well as lacerations to the right eye and the liver.

An employee was collecting trash on the curb when a vehicle drove onto the curb and struck the employee, resulting in skull fractures, a subdural hematoma, and a subarachnoid hemorrhage.

An employee was picking up tree limbs from the side of a road. A skid steer went out of control and pinned the employee against a chipper table. The employee suffered severe swelling, bruising, a broken kneecap, and a back injury.

An employee was standing on the side of a road after a vehicle incident. Another vehicle swerved and hit the employee.

An employee was trotting down the shoulder of a roadway along side the garbage collection truck. As the truck approached a home that had trash out for collection, the employee moved toward the moving truck to retrieve his work gloves. As he reached for the work gloves, his right foot was positioned behind the rear outer dual tire which struck his ankle and fractured it.

On December 8, 2020, an employee exited from the passenger side of the company truck that was parked on the side of the road and walked around the front of the vehicle. As he walked past the truck, he was struck by an oncoming vehicle trying to pass, resulting in a broken left leg.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee was cleaning out an inlet pipe in a stormwater structure. The employee stepped back, fell into the pipe, and landed on concrete 13 feet below, at the bottom of the structure. He suffered a broken back.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was working with paving equipment and heard a noise on the opposite side of the machine he was working on. When the machine stopped, the employee went to the opposite side and crouched down to investigate the noise. A mini track loader backed up and drove over the employee's leg. The employee sustained fractures to their lower leg, ankle, and foot.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
