Final narrative
An employee was placing a piece of plywood on a table saw. As he was sliding the plywood off a cart and onto the table saw, the plywood started to fall. The employee stepped back then fell forward, landing on top of the plywood on the concrete floor. The employee fell on his left hip, resulting in a femoral head fracture that required hospitalization and surgery.
An employee was cleaning paint residue off a protective cover of a machine. The employee was scraping with his right hand when his left hand was caught in a moving chain. The left index finger was amputated at the first knuckle.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
