An employee was using a hammer and chisel to remove a key stock that was fitted/attached between a keyway of a mixer shaft and a mixer gearbox keyway. When the key stock was removed, the mixer shaft spun free and the employee's left hand was caught between the keyways. His left middle fingertip was amputated.
