On May 11, 2023, at 8:15 PM, an employee was unloading a railcar. The car had been steamed and the top was vented. The employee connected the unload hose to the bottom of the car and when the railcar valve was opened to begin the unload process, pressure from the unload piping surged back into the railcar. This caused 200-degree F oil to spill out of the top of the railcar, contacting the employee.

An employee was conducting preventive maintenance on a decline conveyor, spraying lubricant on its tension rollers, when his left arm was caught by the conveyor system and pulled between the roller and the belt. He suffered a fracture to the wrist and two fractures to the forearm.

An employee was bringing a filter online. When the employee opened a valve to initiate flow through the filter, the gasket on the filter housing failed, spraying a water/erythritol mixture (heated to about 175 degrees Fahrenheit) toward the employee. The employee moved back quickly and their chemical suit caught on something, causing it to open. The mixture burned the employee's abdomen and left arm.

An employee was replacing the exhaust fan above the scalding area when their finger became pinched in the exhaust fan shroud. The employee sustained amputation of the distal tuft of their right middle finger with bone loss.

On July 17, 2024, an employee was trying to retrieve his knife from the bottom of a sterilizer. Water in the sterilizer, heated to 180 degrees, entered the employee's right glove and burned his palm and the top of his hand. He was hospitalized.

The employee went to step through the elevator door, but the elevator lift was not there, causing him to fall 20 feet to the basement below. The employee sustained fractures in both legs, the right heel, and lower vertebrae. He was hospitalized.

An employee connected a steam line to a hose to clean equipment when the fitting broke loose. They were struck by steam in the left inner thigh, resulting in burns that required hospitalization.

An employee had turned off the ball valve on a waterpipe system and was removing the plug when the coupling system attached to the strainer came apart. Hot water sprayed on his arm and back, resulting in first- and second-degree burns that required surgery.

Oklahoma Steel & Wire Co., LLC

An employee was using a shovel to remove waste vermiculite from molten zinc. The metal had been placed in a bin and partially hardened. The employee broke through the partially hardened metal; still-molten metal flowed to the employee's steel-toed right boot and entered through the cloth boot tongue. The employee suffered a third-degree burn to the right foot and was hospitalized.

On April 27, 2025, employees were performing a railcar switch. When the train began to shift back, the injured employee's right arm got crushed between the knuckles of two railcars. The employee was hospitalized with fractures to their forearm.

South Dakota Soybean Processors, LLC

An employee was performing routine maintenance on a cracker, which involves removing corrugated rolls and replacing them with new rolls. The employee was putting the machine back together and installing belts and sheaves (pulleys) when a pulley slipped or fell, and caught their fingers against the floor. The employee's right ring fingertip was amputated.

On November 26, 2024, at approximately 7:15 a.m., an employee entered a grain bin to check the quantity left in the bin. They slipped on soybeans and were caught in the moving auger, resulting in lacerations and a fracture to the right foot.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
