An employee was working in the pallet yard stacking lumber when their left index finger was crushed between two railroad ties, resulting in an amputation.

On December 4, 2023, at 12:30 PM, an employee was grinding a pipe. The stops on the skid failed and a pipe rolled backward onto the employee. The employee was hospitalized with bruising and contusions of the right leg.

An employee had just finished inspecting a steel pipe and rolled it forward to begin inspecting the next pipe. The pipe rolled back and caught his finger between both pipes, resulting in a crush injury to the fingertip that required medical amputation.

An employee was using an overhead crane to transfer a flux hopper. While the hopper was being lifted, it swung toward the employee, crushing his left hand and lower arm against a wall. The employee was hospitalized with a wrist fracture.

An employee was working at a grinder when pipes began coming off too fast. As the employee went to stop the pipes with the f-bar, their fingers became caught between the bar and a pipe. The employee sustained left middle and ring fingertip amputations.

On December 19, 2023, an employee was setting up a copper wire drawing machine when his left ring finger was pinched between a copper wire and a machine part. The employee's finger was partially amputated.

The employee suffered injuries to the left index finger, left ring finger, right ring finger, and right index finger; the right index fingertip was amputated without bone loss.

During a routine inspection, an employee placed their hand on a pipe to check a weld. The pipe clamp activated, closed on the pipe, and trapped the employee's right middle finger, resulting in a partial amputation and fracture to the fingertip as well as a laceration to the ring finger.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
