An employee was loading a trailer of automotive parts when the load caught the top of the trailer. He jumped up to clear a wedged item. When the item came down, the load collapsed onto the employee, resulting in a fractured pelvis.

An employee was performing maintenance on an a conveyor system. He was working to loosen the chain that moves the belt when his right index fingertip was caught by the chain and amputated.

An employee was walking on the dock when a forklift ran over their right foot. Their foot was crushed. The employee was taken hospitalized and required surgery.

An employee was cutting corner boards off a pallet with a box cutter. The box cutter lacerated his arm.

An employee was collecting pallets in the freezer when his foot slipped on condensation on the platform of the pallet jack and struck a post, resulting in a foot fracture.

An employee was patrolling a store using an electric patrol scooter when they lost their balance and sustained an ankle fracture.

An employee was operating a forklift to move panels around the dock area when the vehicle struck a joint where the two floors meet. The employee lurched forward and struck his head on the steering wheel. The employee then fell from the vehicle and sustained a dislocated disc in their back, requiring hospitalization.

An employee was standing inside a semi-trailer when the driver of the semi-trailer pulled away, causing the employee to fall backward and strike their head on a steel dock plate. The employee suffered a skull fracture, a fractured L1 vertebra, a broken coccyx, a laceration/abrasion to the head, and a brain bleed.

An employee was riding an ATV, catching up with a stray cow to return it to a pen. The ATV tire struck a hole or other obstacle, causing the employee to be ejected and fall to the ground. His leg was broken and he was hospitalized.

An employee was driving a van in reverse when they fell out of the vehicle and landed on the ground in the parking lot. The employee sustained a fractured left hip.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
