An employee was closing a roll-up trailer door when he lost his footing and the door closed on his right leg. The employee was hospitalized with a fractured right leg.

An employee was performing regular observation of the gate and all approaching vehicles. The weather was hot and the employee was sitting in a vehicle. The employee sustained dehydration and heat exhaustion.

On April 30, 2025, a driver was making local deliveries. At approximately 10:15 AM, while climbing down from the back of the trailer, he lost his balance and fell to the ground from a height of less than four feet. The employee's left leg was fractured and he was hospitalized.

On January 25, 2025, at 6:36 a.m. an employee was opening the door of a trailer at the dock. The 55-gallon drums loaded inside had shifted during transit, causing them to lean against the trailer door. The drums fell when the door was opened, and the employee was crushed. The employee was hospitalized with a broken right tibia.

An employee was using a dolly to hook two trailers together. When she went to move the dolly, she felt pain in her back and suffered a back sprain that required hospitalization.

An employee was driving a truck when it lost power. He pulled over, opened the hood, checked the engine, and opened the radiator cap. Pressure in the radiator caused hot fluid to spew over the employee, who suffered second-degree burns to the left arm, hand, and leg.

An employee was pulling apart pallets that were stuck together when they felt pain in their stomach. The employee was hospitalized with a hernia.

An employee was getting out of a truck tractor when he fell to the ground. He suffered a broken left femur.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
