An employee was loading sand onto a moving locomotive when they jumped off and fell, resulting in a broken ankle.
An employee was standing on a pile of sand when he lost balance. He reached out as he fell and his finger became pinched between a belt and pulley on running equipment. The employee's middle fingertip was amputated.
An employee was removing a rock that had stuck on a dump truck gate. The gate shut on his fingers, causing amputations to three fingers on his right hand.
An employee was climbing up to a machine platform when his left hand came into contact with a conveyor pulley system. Three of his fingers were amputated. The pulley system was unguarded at the time.
An employee fell from the top of an industrial truck, breaking eight ribs and a vertebra in his upper back and suffering a punctured lung. Fall protection was not being worn at the time.
