A temporary employee was cleaning up. As they bent down to pick up a trash pile, a rolling cart rolled into a stack of drywall. The drywall then fell on the employee's right leg, fracturing his tibia.

Two temporary employees were attaching a tow-behind air compressor to a truck for transportation. They had tried to use the trailer's attached jack to lift the compressor, but the jack could not raise the trailer high enough to clear the truck's hitch. Instead, both employees manually lifted the trailer and placed it onto the hitch. During this process, the jack separated from the trailer; when the trailer was set down on the truck's hitch, the jack caught the right middle finger of one of the employees. The finger was amputated at the last knuckle.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The injured employee was putting away materials on the warehouse floor as a co-worker was pushing a pallet onto a nearby shelf. The pallet knocked another pallet forward, causing it to fall onto the injured employee's shoulders. The employee suffered fractures in their right and left knees and ankles.

Employees were moving a single man lift into a building and reclining the lift to position it to fit through the door. The lift shifted and fell, causing the employee to sustain fractures to the left tibia and fibula. The employee was hospitalized and had surgery.

An employee was moving a light tower so it could be hitched to a truck. He grabbed the tongue of the trailer hitch on the light tower to slide it to the left. The tongue jack fell off, causing the light tower tongue to drop on the employee's right hand. The employee's middle fingertip was amputated.

A temporary employee fell from an A-frame ladder to the floor about 5 to 6 feet below. He suffered fractures to his left hip and was hospitalized, requiring surgery.

Epic HR LLC

A temporary employee was operating an electric pallet jack at a loading dock. While maneuvering the pallet jack into a trailer, the equipment loaded onto the pallet jack began shifting to the left. The employee's left foot was caught between a yellow guardrail and the side platform of the pallet jack, and his left ankle was crushed and fractured.

A temporary employee was cleaning a room and compressing the contents of a garbage bin using her right foot. As she pulled her leg out of the bin, she lost balance and fell to the ground, landing on her left hip. The employee sustained a fracture to her left hip and was hospitalized.

An employee was driving an electric pallet jack when it struck a rack. The employee was hospitalized with a crushed and lacerated right ankle.

A temporary employee was filling a sweeper with gas at a construction site. He was shocked by the jumper cables, which caused him to move his right hand into the sweeper's rotating fan blades. His middle fingertip was amputated.

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
