An external contractor was cleaning the floor with a motorized floor scrubber when he smelled propane. The contractor got off the scrubber and opened up the compartment with the propane tank and took it halfway out to check connections. When he tried starting the scrubber back up, the scrubber erupted in flames and he sustained burns. The contractor summoned additional employees. While trying to extinguish the fire with a fire extinguisher, the scrubber exploded again and caused burns to another employee's head, face, neck, and left hand.

An employee was lifting a dropped metal coil off the floor. He suffered a compression fracture to a thoracic vertebra and was hospitalized.

On May 10, 2018, an employee felt a pop in the abdomen while lifting a 50-pound coil. The employee was hospitalized with pain and nausea.

An employee was throwing blankets onto a blanket folding and stacking machine. A blanket got stuck in the machine and she attempted to remove it when the machine contacted her right little finger, resulting in a fingertip amputation.

An employee was pulling a steel parts handcart when its wheels caught on something and locked up. The cart fell backward. The cart handle struck the middle of the employee's right foot, breaking bones. The employee was hospitalized and required surgery.

An employee was cutting wood with a circular saw. The saw kicked back, and its blade cut three fingers on the employee's right hand, resulting in an amputation.

An employee was installing a door from an A-frame ladder. He fell from the ladder, landed on his feet about 8 feet below, and suffered a broken tibia.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
