An employee was working from a bucket truck approximately 20 feet from the ground. While stationary and elevated, the work truck was struck by another vehicle and the employee was ejected from the bucket. The employee fell to the ground and suffered a broken femur and fractures to their hip, spine, and ribs.

An employee was standing on an extension ladder that was leaning against the wall while pulling wire through the ceiling. The ladder and the employee slipped and fell to the floor. The employee was hospitalized with a fractured femur.

An employee was closing a hand hole lid when the lid fell on the employee's fingers. The employee's left middle fingertip was partially amputated.

An employee was sitting in his personal vehicle parked on the side of the road in a road construction zone. A motor vehicle traveling the wrong way on the highway drove into the construction zone and struck the employee's vehicle. The employee sustained two broken ribs and a broken leg.

Highway Light and Landscaping, Inc.

An employee was working out of a lineman truck bucket about 18 feet over a coned-off lane, installing traffic light mounts on an under-bridge deck. A semi tractor trailer swung wide and hit the bottom corner of the lineman truck bucket. The impact pinched the employee's arm between the bucket and bridge, breaking the arm.

Penn Waste, Inc.

An employee was standing on the step on the back of a stopped trash truck. The police were pursuing a vehicle at a high rate of speed when the vehicle hit the back of the trash truck. The employee was thrown from the truck. He stood up and walked for a moment before falling down. He suffered a concussion and neck/upper back strains.

An employee was test-driving a go-cart he had been working on when it struck a parked car. He broke his left ankle and two ribs on the left side.

An employee was in his vehicle going through a green stoplight when traffic came to a complete stop. He was then struck from behind by a van, suffering brain bleeding as well as neck and back injuries. He was hospitalized.

An employee was terminating cables in a junction box. A loose ground wire came into contact with the bushing, causing a flash that burned the right side of the employee's face and his right hand.

An employee was terminating conductors to buss bars inside the secondary compartment of a single-phase transformer. An arc flash occurred, causing burns to the soft tissue of the employee's face.

An employee was securing the claw of a grapple truck to the truck bed. His left little finger was caught between the tie down strap and the rub rail of the truck, resulting in partial amputation of the finger.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
