An employee was in the yard and stepped onto bundles of shingles on a pallet to count inventory. The employee then stepped down from the bundles to the pallet approximately 16 inches. The employee's ankle/foot rolled, resulting in a fractured tibia and fibula as well as a torn ligament.

An employee was stowing the outrigger at the rear of his truck when his right little finger was pinched between the outrigger and the frame of the truck. He sustained a fracture and the amputation of his finger.

An employee was using a conveyor to convey shingles to a roof. The conveyor's chain and sprocket caught the tip of the employee's left middle finger, amputating it through the finger bone.

An employee was on a truck attempting to stow an aluminum extension ladder onto the truck's conveyor boom system. As the conveyor mast was lowered, the boom control area came down on top of the employee's left foot, resulting in two toe fractures.

An employee was unloading roofing products from a flatbed truck when they tripped over a strap on the truck bed and fell to the ground, resulting in a dislocated left ankle.

An employee was walking behind the store with another associate to get paperwork from the delivery driver when she fell on the walkway and sustained a leg fracture that required surgery.

An employee was adjusting the forks on a forklift when a fork detached from the forklift. The employee jumped out of the way but fell on the concrete floor. The employee was hospitalized with a broken hip.

An employee attempted to open a door while carrying packages when they fell and suffered a fractured left hip.

An employee was playing kickball as part of a team building activity when they fell and suffered a dislocated left foot.

An employee was unloading freight from a truck when a stack of doors fell over and knocked the employee to the ground. The employee sustained a fractured femur.

An employee was performing rebar fabrication using a bending machine. The employee's glove became caught in the area where the rebar bend overlaps onto itself. The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.

An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.

An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.

An employee was operating a router when it cut his right little finger, causing an amputation through the bone.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.

An employee was conducting a delivery of beverage products and began having cramps due to dehydration. The employee was hospitalized.
